Please ensure that you arrive 10 minutes early so all Patient Registration and Health questionnaires are completed prior to seeing your surgeon.
We would request that you forward your referral well in advance of your appointment. This allows our staff sufficient time to ensure your doctor will be able to treat your condition. He may recommend you have some tests in advance of your appointment.
You are responsible for ensuring that your referral is up-to-date. A referral from your GP is valid for twelve (12) months from your initial appointment unless stated otherwise.
Please bring the following to your appointment:
- A copy of the referral from your GP or specialist
- Your Health fund, Medicare details or Veterans Affairs Card
- Insurance Documents if MVA or Workers Compensation
- All x-rays, MRI scans or CAT scans that you have had ordered.
Cancellations
If you need to cancel or change your appointment for any reason, please advise reception as soon as possible so we may offer the appointment to another patient.
